O campo organizacional da Saúde em Portugal foi sujeito nos últimos anos a mudanças significativas na sua composição, nas lógicas institucionais presentes e nas relações entre atores e sectores de atividade. O presente estudo baseado numa abordagem metodológica qualitativa, exploratória e descritiva, analisa o sistema de saúde português e a evolução das lógicas institucionais, com a transição do profissionalismo para o controlo do Estado, posteriormente com a imposição legislativa pelo governo de uma nova lógica institucional com a empresarialização dos hospitais públicos e finalmente com a centralização, controlo de gestão e restrição orçamental imposta pela Troika através do programa de estabilidade financeira implementado em Portugal a partir de 2011. Como forma de introdução concebemos dois estudos, o primeiro sobre a evolução do campo organizacional da saúde em Portugal entre 1961 e 2014, e o segundo em que integramos os conceitos de profissionalismo e da teoria institucional para analisar as interações entre os principais atores no campo. Para o terceiro estudo, sobre a intervenção da Troika, concebemos um estudo de caso de dois hospitais públicos portugueses, que incluiu vários componentes e níveis de análise. O modelo conceptual desenvolvido procura analisar as conexões entre os atores principais no campo da saúde e o modo como estas conexões condicionam os processos de mudança no campo. A análise multinível efetuada pretende incorporar uma visão integrada do campo, incluindo as relações entre os diversos stakeholders, de forma a estudar os interesses, os valores, a política e a ação na interação entre os atores.

The organizational field of Health in Portugal has been subject in the last years to significant changes in its composition, in institutional logics and in relations between actors and sectors of activity. The present study, based on a qualitative, exploratory and descriptive methodological approach, analyses the Portuguese health system and the evolution of institutional logics, with the transition from professionalism to State control, later with the legislative imposition by the government of a new institutional logic with the business-like health care and, finally, the centralization, management control and budgetary restriction imposed by the Troika due to the financial stability program implemented in Portugal from 2011. As a form of introduction, we conceived two studies, the first one on the evolution of the field of health in Portugal between 1961 and 2014, and the second in which we integrate the concepts of professionalism and institutional theory to analyse the interactions between main actors in the field. For the third study, on the intervention of the Troika, we conceived a case study of two Portuguese public hospitals, which included several components and levels of analysis. The conceptual model developed seeks to analyse the connections between the main actors in the field of health and the way in which these connections affect the processes of change in the field. The multilevel analysis carried out intends to incorporate an integrated vision of the field, including the relations between the different stakeholders, in order to study interests, values, politics and action in the interaction between the actors.
